City SC Central Valley Waiver & Release of Liability and Photo & Video Consent

Welcome to City SC Central Valley! Please carefully review the following terms, which include both our Waiver & Release of Liability and our Photo & Video Consent. By participating in our programs, events, and activities, you acknowledge and accept these terms on behalf of yourself and your child.

Waiver & Release of Liability

Assumption of Risk: I understand that there are risks involved with my child’s participation in the soccer programs offered by City SC Central Valley. I voluntarily assume all risks on behalf of my child, both known and unknown, arising from participation in these programs, even if arising from the negligence of City SC Central Valley or others.

Medical Authorization: I hereby authorize the directors and staff of City SC Central Valley to act on my behalf in the event of a medical emergency, should they deem it necessary. I understand that I am financially responsible for any medical expenses incurred due to injury or illness arising from participation.

Release of Liability: I release and forever discharge City SC Central Valley, its staff, coaches, volunteers, and agents from any and all liability, claims, demands, or causes of action for personal injury, infection, property damage, or other loss suffered by my child in connection with participation. This release applies to myself, my child, our heirs, executors, and assigns.

Communicable Disease Notice: I understand that my child may be exposed to communicable diseases, including COVID-19, while participating in City SC Central Valley events. I accept these risks and release City SC Central Valley and its affiliates from any liability associated with potential exposure.

Photo & Video Consent

Consent to Use: By registering for City SC Central Valley’s programs, you grant permission for City SC Central Valley to use photographs and videos of your child for promotional purposes, including but not limited to use on our official website, social media platforms, marketing materials, and other public channels.

Respectful Usage: We are committed to using photographs and videos in a respectful manner that aligns with the values and principles of City SC Central Valley.

Public Access: Please be aware that images may be accessible to the general public on platforms such as our website and social media.

Withdrawal of Consent: Your consent is entirely voluntary and may be withdrawn at any time. To request the removal of specific images, please contact City SC Central Valley directly.

Release of Liability for Visual Content: By providing your consent during registration, you release City SC Central Valley, its staff, coaches, volunteers, and agents from any claims, liabilities, or demands related to the use of photographs and videos.

Acknowledgment

By proceeding with the registration process, you confirm that you have read, understood, and agree to the terms outlined in this Waiver & Release of Liability and Photo & Video Consent. This acknowledgment applies to both yourself and your child, and it signifies that you voluntarily accept these terms.
